Medford Youth Hockey and Girl's Hockey Travel Team Coaches, In-House Team Coaches, In-House Hockey Instructors

Thank you for your interest in becoming a Medford Youth Hockey and Girl's Hockey Head Coach, Assistant Coach, or Instructor. There are steps required by Medford Recreational Hockey Association (MRHA) and USA Hockey each year for returning and new coaches to follow for certification. 

MANDATORY REQUIREMENTS FOR ALL COACHES AND ASSISTANT COACHES:

1.  USA Hockey and Massachusetts Hockey require every Coach, Assistant Coach, and Instructor to submit an online NATIONAL BACKGROUND SCREENING FORM (Formerly a CORI Check) administered by NCSI.

2.  USA Hockey and Massachusetts Hockey require every Coach, Assistant Coach, and Instructor to take an online USA HOCKEY SAFE SPORT COURSE administered by USA Hockey relative to on-ice and off-ice player safety. Forms, courses, and additional information can be found here:https://www.mahockey.org/coaches

1.  Register with USA Hockey: www.membership.usahockey.com
2.  Complete the required background screening: www.usahockey.com/backgroundscreen
3.  Complete the required SafeSport training: www.usahockey.com/safesporttraining
4.  Complete the appropriate Age-Specific Training Module (if necessary) for the level of play that you are coachingwww.usahockey.com/agespecificmodules

Notify our Registrar via email at medfordyouthhockey@gmail.com upon completion of these requirements. Please include your USA Hockey Registration ID. MRHA asks that these steps be completed prior to the start of the season.

Steps to use the Fill & Sign Tools:

1. Download the application here.

2. Open the downloaded file on your computer. It should open using Adobe.

3. Once in Adobe, click "Fill & Sign" in the upper right hand section. This will open a field where you can add text, check marks, 

  and signatures.

4. Once you have completed the form, you can save it and send it as an email attachment to board@medfordrechockey.com
